HISTORY OF PRESENT ILLNESS:
This patient had the syndrome since he was born (16 December 1984). Expose to the radio-ART-tive since kid until now.

PAST MEDICAL HISTORY:
Got the first “life support device” in August 2006 make the patience stable and more passionate. To make sure the patient happy and enjoy, he need to go to street photography shooting. Accompanied or alone doesn’t matter. Exposure to the great moments and “freeze” it. Had a lot of Wedding-logy therapy to increase the confident and professionalism.

REVIEW OF SYSTEMS: SYSTEMIC:
The patient have the “life support device” called Canon DSLR EOS 350D companied with Canon Speedlite 580EX, Canon IS 55-200mm and Canon 50mm 1.8 prime lens

VITAL SIGNS:
Candid style, professionalism, confident and a bit crazy.

EXTREMITIES:
Always go for moments, no matter how bad the condition is, still manage to capture good pictures. 1st Prize Winner for Tourism Malaysia Model Shoot-Out 2006. Can cope with Photoshop-therapy

FINAL IMPRESSION:
Creative, think over-under-though-outside the box.

DISPOSITION:
Admission to observation. The patient’s condition is good. He is photography-cally stable
